Online registration for Cosmetex 2013 is now closed. You can still register on site at the Registration Desk in Melbourne Room 1 & 2 Foyer at the Melbourne Convention Centre.

Please note:
All prices are in Australian dollars (AUD) and include GST.
A $20 fee applies to every Manual Registration received as indicated above.
Registrations made after Wednesday 3 April 2013 will incur a $50.00 LATE registration fee.

** $30.00 from each ticket sold, will be donated to a charitable organisation.

Early Bird Registration fee is available until Thursday 17 January, 2013.
To receive the early bird rate you must register online and pay by credit card at the time of booking. Early bird registration closes on Thursday 17 January, 2013.

Full Registration fee is available from Friday 18 January, 2013 until Wednesday 3 April, 2013.
Payment for this registration MUST be received via email or fax by 1700 or post-marked on or before Wednesday 3 April, 2013. After this date the Standard rate expires and the LATE fee will be applied automatically to all unpaid registrations.

ACCS Fellow, Affiliate Nurse / Member Registration standard fee is available from the Friday 18 January, 2013 until Wednesday 3 April, 2013
These registration fees are for an ACCS Fellow, Affiliate Nurse / Member.

Payment for these registrations MUST be received via email or fax by 1700 or post-marked on or before Wednesday 3 April, 2013. After this date the Standard rate expires and the LATE fee will be applied automatically to all unpaid registrations.

Single Day Registration fee is available until Wednesday 3 April, 2013.
Payment for a single day registration MUST be received via email or fax by 1700 or post-marked on or before Wednesday 3 April, 2013. After this date the LATE fee will be applied automatically to all unpaid registrations. Please note this registration fee does NOT include social function tickets.

NOTE: You can ONLY nominate one day to attend, if attending several days, please select a full conference registration.

2 Day Paramedical Program Registration is available until Wednesday 3 April, 2013

Please note this is access to the Paramedical Program Only (Friday and Saturday). There is no access to the Scientific Program or workshops with this registration Type.

CPD Points

Instructions on how to Obtain CPD Points
Delegates wishing to obtain CPD points can self-claim, through The Royal Australian College of General Practitioners, by self-recording based on the following guidelines.

What is self-recording?
A GP can register a total of 20 Category Two points (Capped at 20 points for the triennium).

The QI&CPD program recognises the benefits for GPs participating in events, which may not be formally accredited by the QI&CPD Program, for CPD point allocation that they find personally valuable for their individual learning needs.

After logging in to the RACGP website, use the 'self record your activities' option that is available in the main side menu. This online notification should be updated as unaccredited activities are completed. Once a total of 10 hours of unaccredited activities has been entered, an option to submit the self-recorded notification to the QI&CPD Program will become available. Submitting the notification to the QI&CPD Program is capped at 20 Category Two points towards your total triennium requirement.

Delegates also have the option to complete an Individual Active Learning Module (ALM) (40 Category One points), if you consider this activity meets the criteria, of a pre-disposing activity followed by a minimum 6 hours of interactive education and a reflective activity.

PLEASE NOTE: CPD points can only be claimed based on attendance at the scientific program sessions. Sponsored workshops do not attract CPD points.

Bank Transfer
If you would like to pay your registration fee by bank transfer select the "Please email me an invoice" option on the payment page of the online registration form and then email the Registration Manager at cosmetexreg@cosmetex.org to receive the bank details and instructions.

Cancellations and Amendments
Registration and/or accommodation cancellations and amendments cannot be accepted over the telephone. Alterations and cancellations must be sent in writing to the Conference Organisers, International Conferences & Events (ICE) Pty Ltd.

Refunds
Registration cancellations received up to Saturday 16 March, 2013 will receive a full refund, less $150.00 administration fee and is payable upon cancellation. Registration cancellations received between Sunday 17 March, 2013 and Monday 1 April, 2013 will receive a 50% refund. No refunds will be given for registration cancellations received after Monday 1 April, 2013. As an alternative to cancellation, registrations may be transferred to another person before Wednesday 27 March, 2013 without receiving any extra cost penalty. All amendments, including transfers, made after this date will incur a $20.00 additional fee.

Accommodation
Accommodation will not be reserved without credit card details. Accommodation cancellations less than 30 days prior to the conference will be charged by the hotel according to their cancellation conditions

Conditions
International Conference & Events Pty Ltd reserves the right to deny entry and/or terminate the registration of any person who demonstrates behaviour that International Conference & Events Pty Ltd deems to be inappropriate and / or a risk to others. Registrations received by International Conference & Events Pty Ltd will not be considered as confirmed unless payment has been received and processed by International Conference & Events Pty Ltd. Non-payment or non-attendance will be taken as a cancellation on the day of the event and will incur a 100% cancellation fee.

Disclaimer
The speakers, topics and times are correct at the time of printing. In the event of unforeseen circumstances, the Organising Committee reserves the right to delete or alter items in the Conference Program.

Liability / Insurance
In the event of industrial disruptions or natural disasters, the Australian College of Cosmetic Surgery (ACCS), and International Conferences & Events (ICE) P/L cannot accept responsibility for any financial or other losses incurred by the delegate. Nor can the ACCS and ICE P/L take responsibility for injury or damage to persons or property occurring during the conference or associated activities. Insurance is the responsibility of the individual delegate.
